If you’ve been off sick for more than 7 days

To request a sick note:

Self Certification Sick / Fit Note for less than 7 days

If you have been ill for 7 days or less you don’t need to see a Doctor. You can complete a Self Certification form yourself. However some employers insist on a Doctor’s note regardless. A Doctors note under these circumstances may involve a charge.

Many employers have their own self-certification forms. If your employer doesn’t have its own form you can download the Self Certification Form. Please print it, fill it in and hand it in to your employer. You do not need to see a Doctor.

Fit Notes (Sick Notes) – Important Information

Fit notes are issued once, based on the information provided at the time of request and the GP’s clinical judgement.

Whether you are requesting a new fit note or a repeat, you must ensure that all details are correct and complete before submitting your request.

For repeat requests, you must include:

  • The start and end dates of your previous fit note
  • The exact wording or advice previously issued

We do not amend or re-issue fit notes due to missing or incorrect information provided at submission.
If the information supplied is incomplete or inaccurate, we will be unable to issue the fit note and you may be asked to submit a new request or arrange a clinical review.

Please note:

  • Responsibility for providing accurate information rests with the person submitting the request
  • GPs are not able to guess, recreate, or retrospectively alter fit notes
  • Fit notes are issued at the GP’s clinical discretion and only when clinically appropriate
